What is Chronic Care Management?
• Chronic Care Management provides you frequent follow-up care outside of office visits to closely manage your chronic conditions.
• Your personal care manager will call you monthly to oversee your care including: assistance with med refills, correspondence with your specialists, scheduling appointments, and being pre-triaged, saving you time in office.
• We help motivate you to achieve your desired quality of life. The goal is to keep you healthy and at home.
How will you benefit from Chronic Care Management?
• Greater access to care. You have direct access to your primary care team, who will be spending more time coordinating and managing your care to make sure nothing slips through the cracks.
• More preventative care. We will more actively monitor your health and needs to catch problems before they arise, keeping you healthy and at home instead of the hospital or doctor’s office. Preventive care services will be scheduled for you and your medications will be closely monitored.
• Access to your health records. You will receive a personalized, plan of care for your health shared with your other health care providers.
What do you need to know before signing up?
• Medicare requires a copayment, usually below 10 dollars month, which we can waive if a financial hardship exists.
